1. Registration Form
2. Waiver of Liability
3. Consent to Treat

Step 2:
Fill out the forms completely for each child you would like to register. Don't forget to sign them!

Step 3:
Bring the completed forms to Registration Night at the Waseca Community Arena on:      Monday, August 27 from 6 to 8 PM
